Employers:

C2 - Employer's Report of Work Related Injury/Illness

Form C2 is used by employers to report workplace injuries or illnesses to the New York Workers' Compensation Board. It includes details about the employee, the nature of the injury or illness, and the circumstances of the incident. Timely filing of this form is critical for ensuring compliance and initiating the workers'

C-11 - Employee's Change in Employment Status

Form C-11 is an Employers report of injured employee's that results in a change in employment Status resulting from that injury. 

C-107 - Employer's Request for Reimbursement

Form C-107  allows employers to seek reimbursement for payments made to an employee during a Workers' Compensation claim. This form is used to recover wages paid while the employee was out of work due to an injury or illness.

C-240 - Employer's Statement of Wage Earnings

Form C-240 is used by employers to report an injured employee's wage history to the Workers' Compensation Board. It helps determine the compensation benefits the employee is eligible to receive.
